How to Prepare for Your DOT Medical Exam

Here is the medical documentation that is required for BarnesCare to complete your DOT physical exam.

  1. Are you currently taking medications (prescriptions, over-the-counter, herbal remedies, diet supplements)? If so, please list them on your long form.
  2. Do you wear glasses or contact lenses for distance or driving? If so you will need them to complete your exam.
  3. Are you diabetic, borderline diabetic or do you take medication for diabetes? If so, we need documentation of your HGBA1C results within the last six months. Please note that the level must be below 10%.
  4. Are you taking insulin? If so, the provider prescribing your insulin must have completed the required form MCSA-5870 within the past 45 days. Ask our staff if you do not know how to obtain this form, or it can be found by an internet search.
  5. Do you have sleep apnea or have you ever had a sleep study performed? If so, we must have a clearance statement from your sleep specialist. Or if you use CPAP we need a compliance report covering the period since your last DOT exam (must be 70% at least four hours per night).
  6. Do you take blood thinners, such as Warfarin or Coumadin? If so, we must have documentation of INR regulated for the last month.
  7. Have you been diagnosed with heart disease, heart attack or had PTCA/stent procedure? If so, we must have an annual letter from your cardiologist and a copy of your cardiac stress test results within the past two years.
  8. Have you had heart surgery, valve replacement or diagnosis of heart failure, we must have a report of an annual echocardiogram and an annual letter from your cardiologist clearing you to drive a commercial vehicle.
  9. Do you have a pacemaker? If so, we will need documentation of your annual pacemaker check.
  10. Have you been diagnosed with seizures or epilepsy? If so, we will need documentation of you being seizure-free, without taking seizure medication for 10 years.
  11. Do you use hearing aid(s)? If so, you need them for testing today.
  12. Have you been diagnosed with an attention deficit disorder (ADD or ADHD)? If so we need a one-time letter from your private health care provider clearing you to drive a commercial vehicle.
