To expedite patient registration and treatment services, a completed Medical Authorization Form is required for all patient visits. The completed authorization form identifies client company and the individual presenting for services. It also allows you to provide specific instructions or goals of the visit. 

“Without a completed authorization form, we have to stop the patient registration process to contact the employer by telephone,” says Debbie Teeter, PTA, BarnesCare Midtown supervisor. “This causes a delay in treatment and can be frustrating for both the patient and employer.”
