These reports do not replace the personal communication of our BarnesCare physicians and staff. Our physicians and therapists are always available and willing to discuss your employee's care, and answer any questions or concerns you may have.
If you have a case that is going to litigation, we recommend you have your legal representative request a complete and all-inclusive medical record from the BarnesCare location that provided the patient's treatment.
Visit Summary
. Summarizes a patient's visit, i.e., date and time patient arrived, time departed, date of injury, name of treating physician, diagnosis, plan of treatment, tests ordered, medications dispensed if any, and follow-up instructions. A copy of this report is given to the patient at time of departure for his/her supervisor. If requested, this report can be faxed automatically to employer (and insurer/payor, if requested) immediately.
Physician First Report
. Includes all of the information in the Doctor Visit Summary, but also includes patient's history of injury/illness and permanency information. A copy of this report is mailed to the employer (and insurer/payer, if requested) within 24-48 hours. If requested, this report may be automatically faxed to employer (and insurer/payor if requested) immediately.
Discharge Summary
. Summarizes patient's discharge from BarnesCare and any continuing follow-up care that is needed. A copy of the report is given to the patient at time of departure for his/her supervisor. If requested, this report can be faxed automatically faxed to employer (and insurer/payer, if requested) immediately.
Physical Therapy Notes
. Summarizes physical therapy visit and plan of treatment. A copy of this report is sent to the employer (and insurer/payor if requested) after every visit. If requested, this report may be automatically faxed to employer (and insurer/payor if requested) immediately.
Closed Case Analysis
. A detailed report on all closed cases that compares a specific company's injury and outcome data versus the BarnesCare client average. This report provides detail on lost day cases; average number of visits per case for injury treatment and rehabilitation; average total clinic time; and percent of cases that required rehabilitation, prescriptions and referral to a specialist. This report must be requested by the client company or insurer.
